WASHINGTON (WUSA9) -- A boy from Virginia who was diagnosed with neuroblastoma and started a non-profit called Trevor's Treasures has died, according to a journal entry from the Trevor's Treasures Twitter account.

We told you about Trevor Blake of Woodbridge, Va. in March 2012 ( Blake was diagnosed with the rare and incurable cancer at age five, and at the time of our report in 2012, he was age 10. Only 10% of children diagnosed with this form of cancer survive for five years or more.

Trevor Blake got the idea for Trevor's Treasures because his dad would bring a bin of toys and let Trevor pick out a couple of his favorites when he visited Trevor in the hospital. Trevor's Treasures delivers toys to other child cancer patients and also teamed up with the annual Hoops Fest fundraiser.

On Sunday, @TrevorsTreasure tweeted a screencap of a journal entry. In the entry titled "Trevor my Angel," his mother wrote:

"My strong brave boy has received his wings. With dad, Mom, dad #2, and Grandma Blake by his side he has moved on to fulfill his mission of watching over all of us and all the other children faced with battling this awful disease. He has won. He has inspired so many and will carry that inspiration on for eternity. So here we all are, its time to pull a Trevor. As upset as we are, it is time to move forward and carry on with the lessons he has taught us. Selflessness. I love you Trevor. With all my heart. You are and will always be my strong brave boy."
